Scholarship Application Form

The Change for Youth Foundation currently subsidises 1/3 of the costs of the Empower Youth program. Limited full scholarships are available thanks to the generosity of our supporters and donors. However, we kindly ask that you exhaust all funding options through the Complex Support Needs Allowance, High Support Needs Allowance, Child Related Costs, and other funding sources before applying for a scholarship.
